Volunteer covid-19 Agreement

For in-person classes, I agree to abide by DFD's COVID-19 policy including, but not limited to the following:

• I/volunteer will not enter the building or come to class if we're exhibiting any symptoms or have been exposed to COVID-19.

• I/volunteer will notify DFD if anyone in our household has tested positive or shows symptoms of COVID-19.

• I/volunteer will quarantine for a minimum of 10 days after initial exposure, symptoms or positive test result.

• I/volunteer agree that anyone coming to a studio will wear a mask at all times.

I certify that I am at least 18 years old as the volunteer or that I am the parent or legal guardian for the volunteer and have the legal right to waive these rights.
